How do I add a new patient?

00:00: This tutorial guides you through creating a patient record in amp health.

00:04: Click the Patients tab to access the patient roster.

00:08: Click the add patient button to start creating a new patient record.

00:14: Click the first name field to enter the patient's first name.

00:18: Enter the first name in the designated field.

00:20: Click the Last Name field to input the patient's last name.

00:24: Enter the last name in the appropriate field.

00:27: Click the email field to provide the patient's email address.

00:31: Enter the email address in the email field.

00:36: Select the Clinic from the municipality dropdown

00:40: Click here to select the patient's gender.

00:43: Select the patient's gender.

00:45: Click the date of birth field to specify the patient's birth date.

00:49: Enter the correct birthdate for the patient.

00:51: Click Yes to invite the user via email, or No to create the patient without inviting them.

00:57: Click the Complete button to finalize patient creation.

01:01: Click Assign Physicians.

01:03: Select appropriate Physician form the dropdown menu.

01:08: Click Assign Physical Therapist

01:10: Select the Appropriate physical Therapists from the dropdown menu.

01:14: Select the toggle to configure the episode of care.

01:18: Click Proceed

01:20: Select the affected joint from the dropdown menu.

01:23: Select the affected side from the dropdown menu

01:27: Select injury status.

01:28: If Yes, sescribe the injury

01:31: Confirm that the patient is having surgery.

01:34: Click the Surgery Date field to specify the date of surgery.

01:39: Click here to open the surgery. Date picker.

01:42: Click the 14th to select the surgery date.

01:45: Select the appropriate procedure.

01:48: Toggle the appropriate restrictions.

01:50: Click the desired restriction.

01:53: Add a description.

01:55: Click days to specify the duration of the restriction.

01:58: Enter the number of days for the Restriction period.

02:01: Click here to open additional restriction options.

02:05: Continue the process for each restriction. This information will also be displayed for the patient.

02:11: Click create episode to save the new episode details.

02:15: The new patient profile is now avialable.

02:18: You have successfully created a patient record in amp Health by entering personal details, medical history and episode information. Next you can manage patient treatments or update records as needed.
